Information needed to start your Social Security disability claim includes:

• a list of physicians and other medical providers who you have seen over the past 15 to 20 years. This list needs to include a current address and phone number
   
• a list of hospitals where you have been seen over the past 15 to 20 years. This list needs to include a current address and phone number
   
• a list of your past jobs over the past 15 years. Wherever possible, you should provide a current address for the employer and a brief description of the type of work you did
   
• a list of your current medications
   
• a list of people who know you well and who would be able to speak about your medical or mental health condition
   
• a list of the schools you attended and a brief description of your performance at those schools
   
• a brief description of your medical problems
   
• a brief statement as to why you believe that you are no longer able to work

I recommend to my clients that they write out this information neatly and keep it handy. You will be referring to and updating this information throughout the course of your Social Security disability claim.
